Family Pre-Registration

The registration form below is designed for you to tell us more about your child(ren) and so that we have their emergency contact information. The form below is to be completed for any children or youth participating in:
1. Godly Play
2. Bible Explorers
3. Youth Group

The form needs to be completed just one time for the 2023-2024 school year (one per family). Only one form needs to be completed for any children in both Godly Play and Bible Explorers or youth participating in both Bible Explorers and Youth Group.
